If you could implement a body with a pair of arms holding in one hand a cane and in the other hand a jar of ritalin then then you'd really have some quality shirt. The head however I wouldn't change because the yes because the eyes give a great look into what's going on in his head.

two things i'd change. the bill of the hat looks really strange, i think i'd make it wider. and the four round flat pills at the top left are all facing the exact same direction.
