In this pic Rosemary shows more signs of discontent in her surroundings. She wishes so much to be happy that in her delusional mind, she sees a fairy smash through the window (that isn't really there) and offer her a pebble from outside, reminding her that there is still a world beyond these four walls.
She barely looks human because thats the way she see's herself. She feels more alien than girl at this point in her life, locked away in an asylum and treated like a science experiment by the doctors.
